How do I fix DeviceTwin "connectionState": Disconnected ?

When working with Azure IoTHub, occasionally I have issues with the DeviceTwin not connecting as shown below:

Right now, the only workaround I've found is trying to completely recreate a new device - but I'm assuming there must be a way to fix this instead of doing that.
What causes the DeviceTwin to disconnect, and how do I rectidy it?
