I have seen similar issue recently. Check appenforce.log to verify if the app is installing or not. In my case it was, but the step was reporting with error 16389 on CB 2010. In my case the issue is random.

We may check smsts.log and appenforce.log for more details.
And what is the detection method are you using for the application? If it's powershell script, that might cause the error code 16389.
