What happened to my ad fs sign-on page images?

Greetings all,
we've implemented AD FS Single Sign-On and the Sign-On page has no images. It's just a white page with the following text in the upper left corner:

Single Sign-On
Sign in with your organizational account
User Account
Password
Keep me signed in
Sign in
2016 Microsoft

The first line is being pulled from the Federation Services properties "General" tab "Federation Service display name" field. The other lines contain the appropriate field window for text entry and/or checkbox. What happened to the default Theme that ships with Server 2016? I thought it might be getting stripped by some group policy but I tested with a stand-alone workstation and got the same results. These are the results using Firefox. In IE, I get the intended destination application page, all white with just its copyright and info links at the bottom of the page.

What gives?

    Could you share a Fiddler trace from one of the clients?

    Sorry, Piaudonn,
    It's against policy to use Fiddler or transfer data from the system.
