Forms QR code doesn't always work

I'm using some forms to automate taking attendance in a class. There's a one question quiz, all the answers are jokes so it doesn't matter which they choose, but they are logged in with their institutional account so it collects their names and the date and time. I project a QR code at the beginning of class that takes them to the form.

For 75% of students, this works fine. For the other 25%, it returns an error message saying there is no such form. I can't figure it out. I've watched them do it and they are logged in to their institutional account so it should not be blocking them for that reason. If Forms doesn't work on something simple like this, I certainly will not be inclined to use it for anything more important.

    Apologies for the inconvenience caused and please don’t worry, let’s work together on your concern and move towards a resolution path.

    I understand your concern and to my knowledge, I would like to convey that the problem facing with 25% of students is not an expected behavior when scanning the QR code generated for the active form/quiz in the Microsoft Forms.

    Since they are getting there is no related Form exists, I would like to request you ask the problematic students to click on the Copy URL instead of clicking Open in browser after scanning the QR code of the Quiz, here are the steps to try:

    Scan the QR code of the Quiz on the problematic mobile device> Click on the Copy URL> Open the Google Chrome browser> Click on the three dots at the top right> Click on the New Incognito window> Paste the URL and check the outcome.

    If the same problem persists, I would like to confirm the following information which will help us to better understand and guide you understanding:

    1. Does the problem happen with the specific Quiz or all Quizzes for the problematic users?
    2. If you share the response link of the Quiz instead of QR code, do the users have the same issue?
    3. Does the same problem happen when the problematic students access the Quiz in the different device with their account?
