azure devops testcase suite incident created duplicate testcase getting created without linked to userstory

azure devops testcase suite - adding userstory into it and then adding testcase using testcase grid -> test is getting added but

duplicate testcase is also getting added without linking the userstory. Is this an know issue in azure devops. Please suggest.

Yes, manually we are adding from azure Test plan->Adding userstory as requirement First-> from there..

Second we add Testcases using ->Add testcase using grid ...It create the testcase for the requirement userstory ->

but it also create an duplicate copy of testcases without linking the userstory.

it happen quite number of times..its create duplicate copy without linking to userstory...

from this testcase grid view..it should alway link the testcases to userstory..but somes it create duplicate copy without linking to userstory..

Please see as an example of testcase grid view for reference..Please suggest if this is know issue in azure devops.
