Create a flow through Power Automate to send personalised email when certain conditions are met in a SharePoint List

Hi Team

Can someone outline the steps to show me how to create a flow from a modern SharePoint list to an email through outlook when the below conditions are met;

when the change status = "Triage in Progress" & the Go Live date is less then 30 Days for a personalised email to be sent to the "Project Contact" requesting more information

Below are the fields in the SharePoint list

Change Status (Drop down option)
Go Live Date
Project Contact= is a free text field with the contact's name.
