NPS not supplying the VLAN information to the switch

Hi guys,
Im trying set up 802.1x vlan via NPS.
When I plug in my machine it seems to authenticate however its not getting the right IP address.
I only get IP address from default vlan 1.
What am I doing wrong ?

I looked at this issue from the switch side and it shows that the switch did not receive vlan info from the NPS.
We did a pcap further on the NPS to check if the ACCESS-ACCEPT message includes the Tunnel-Pvt-Group-ID as one of the AVP, but as shown in the screenshot, the NPS is not providing the VLAN info in the Radius message.
Any help on the server end will be much appreciated.
