User group failed to attached after granted the access from package

We linked a security group to a access package , so that user are automatically added if the approval granted.
We recently did some testing on it and find out some account are not added to the security group even the access granted .
test users even received an access confirmation email of the package from MS .
Can you please advise is it an error or we did something wrong ?

In addition, you need to confirm that you have met the prerequisites here.
