The organization chart is created automatically when you define the manager of an identity.
So, in your example, department A should be something like this:
- Senior Officer, His/her manager is the director
- Officer, his/her manager is the Senior Officer
- Receptionist (1), his/her manager is the Senior Officer
- Receptionist (2), his/her manager is the Senior Officer
To add the manager for your Azure AD, you can follow this document https://learn.microsoft.com/en-us/azure/active-directory/fundamentals/active-directory-users-profile-azure-portal
I you have an Azure AD Connect, you must edit the Manager Property on your Domain controller
