Looking for a product to create a form flow to a smart report. For example: Plaintiff Name: Mary Jones , everywhere the word plaintiff is in the questionnaire or form should be prepopulated with Mary Jones . I am very familiar with using Conditional Logic to show or hide fields. The questionnaire once completed needs to print only those fields that have been checked or data entered into that field. Some paragraphs will print on all documents, some will only print if checked. Basically need to take a 12 page questionnaire and print a user friendly legal document that can be edited/updated at a later date. Any assistance would be greatly appreciated.

Hi @ Bert Barber I am responsible for SSRS related issues. Do you want to do a survey? If you want to make a sample questionnaire and print it, then SSRS can do it. But if you want to collect survey information from many users online, you can't do it. SQL Server Reporting Services (SSRS) provides a set of on-premises tools and services for creating, deploying, and managing paginated reports. It imports data from data sources, giving it the flexibility to deliver the right information to the right users. It's not really a survey. But more check the box(es) and fill in the blanks. Then print a document containing those fields along with other paragraphs associated with those check boxes or filled in blanks. The printed document needs to ready like a legal document not a report format.

For example:

Fill in the blank - Plaintiff: Mary Jones

Check box - Plaintiff prefers to be referred to as: Mother / Father / Grandmother etc. = Mother

Fill in the blank - Child(ren) under age of 16 - Melissa L. Jones DOB 12-1-1999

Document should read as follows:

The plaintiff (plaintiff only prints 1 time) Mary Jones (should auto populate everyone in the from the word plaintiff is found) is the mother of Melissa L. Jones minor child born 12-1-1999.

Has custody from (fill in the blank) Friday, August 1, 20?? till Sunday, August 3, 20?? (dates are pulled from the form fill in the blank boxes.

So, I am trying to take a form with multiple fill in the blanks and check boxes and turn it into a report using those data fields.

If you need to set multiple check boxes in the body, then SSRS may not meet your requirements, it only has check boxes for selecting parameters. According to my understanding, Power BI may realize your request, you can go to related platforms for help: https://community.powerbi.com/.
