If I understand you correctly, you have reached the limit for the complete azure tenant scope (2500). This is a fixed limit and cannot be changed.
Is it possible to combine the initiatives?

In this case, the best option is to use the REST API or Powershell (Get-PolicyAssignments)to query the policy assignment endpoint and compare active assignments against all PolicySets (Get-PolicySetDefinitions). The difference represents the unused PolicySets.
Is there an effort from Azure Policy PG to perform such actions across Microsoft tenant portfolios?
On our end, we are using all of our initiatives, and following limit blocks our deployments (division wide).
Unfortunately not, you have to do this via the descriped approaches.
As described previously, I am unable to do the cleanup on my existing Initiatives since they are all in use. Please direct me to the right place to raise an incident/feature request to increase the limit or perform a cleanup. Thank you.

The 5500 limit of Azure Policy Initiatives was hit.
I am unable to create any new initiatives and our scenario doesn't support combining the old ones.

What do you mean by un-used/tested/forgotten initiatives? As I understood you, all initiatives are in use. In this case, it is technically difficult to filter such un-used/tested/forgotten ones. This would have to be evaluated and decided manually for each initiative individually
I recommend opening an issue at Microsoft with reference to the limit.
