How to create a ticketing system with SQL Server and MS Forms and autofill MS Forms from server

I want to build a ticketing system using MS Forms that use SQL Server as the backend. I want to autofill data from the server. Suppose you type in your name, department, phone number, supervisor, etc., and auto-populates. I would then like the data from SQL Server to create reports in Power BI.

If this doesn't work, what will? I need to keep the tables on SQL Server.
