We're currently in the process of expanding the capabilities of our app. While manually adjusting the number of instances works correctly, we can't get the automatic scaling option to work. Unfortunately, any combination of settings we've tried has only led to a vague error message stating, "The scale out operation for the app 'xxxxxxxxx failed: An error has occurred."
We're wondering if anyone else has come across this issue?

Same here. I'm facing this issue with one app service only - Automatic scaling.

No solution yet and unable to identify rootcause

Fix has finished from being deployed. At this point this should have been fixed for all regions.
