Could be some sort of file system corruption. chkdsk may be needed but given the issues I'd back it up before doing so.

I have experienced an unexpected incident of a missing Virtual Hard Disk (VHD) that occurred during the timeline of a single faulty hard disk within a RAID 5 configuration. As RAID 5 provides fault tolerance for exactly one hard drive failure, so I am puzzled as to why this incident has resulted in a missing VHD.
I have been in touch with DELL Support to verify that the issue is not stemming from the hardware. After conducting a thorough health check and analysing the server logs, DELL has confirmed that there is no issue on the hardware side. They have suggested that the issue likely lies at the operating system level.
Note: The disappeared VHD is still taking up the space in the disk

Operating System: Windows Server 2019 (10.0)
