adding a new authenticator 2fa does not show up on authenticator app

I am running v. 6.8.2 of authenticator on iphone 13 v. 17.1.2

I am trying to add a new 2fa account to the authenticator app.

The first time I scanned the QR code to add the 2fa and it was successful, but did not see the 2fa number on the main authenticator screen. I thought maybe it didn't work. I tried to scan the QR code a second time and I get a message that I will be replacing existing 2fa. I go ahead and still do not see the 2fa code.

I have multiple (5) 2fa profiles and all worn on authenticator, just this new one does not work. any thoughts?

is there a limitation on number of 2fa items?
