The form states:
Updates about your onboarding request: Please add maccount@microsoft.com and csgate@microsoft.com to your Safe Senders list to ensure you receive all correspondence pertaining to your application, including the verification email from Microsoft Vetting Service. For questions about the application process, please contact our team at csgate@microsoft.com
The application (from what I have seen) is a lengthy process and can take some time for the Azure OpenAI team to approve your request.
