Today we are facing a problem with the access to database, only we can not access by aplications or direclty into the azure portal only we are getting the message that database is not currently available, do you know what we can do? I can not see any option to restart or do something in the portal

Just to check, is your server in East US region? If yes, seems be some issue in that region. You can open a support case to check the health of your server and platform side issue.
Also, please check the Resource Health on Azure Portal. It is located on the left vertical panel related to your Azure SQL database.

I did a manual failover and it started working. I am new to Azure. Do I have to do failover again or I can continue with secondary DB as my primary now?

Yes, the issue is mitigated. If you have a geo replica and had failed over to the geo secondary during impact, you can now fail back to East US.
