Custom question and answers not found in SharePoint list using Power Automate

Question

I am struggling with custom question and answer set up in a flow that works perfect until the SharePoint create item action. It is not recognizing the custom question and answer in order to populate in the SP List. The custom questions are 1. Case Name and 2. Case Number. Would be grateful for some assistance
