Identity validation fails with "Please provide additional document(s)" but there is no way to provide the documents

I am trying to create a new public identity but I get a failure asking me to "Please provide additional document(s) dated within the past 12 months to verify your association with the organization. Examples of verification documents are as follows:" There are no examples of verification documents and there is no way (that I can find) to provide the documents.
Not sure what to do now.
