How are you checking the profile card? Keep in mind that "classic" desktop apps do not support this feature, so it's best to check via the web apps. Or the mobile ones. Teams should also be fine. And, the process is far from instantaneous, it can take days.
Custom attributes are not showing in User Profile Card
We tried to add the 'custom attributes' property in the profile card in our tenant using the profile card API in MS Graph. It seems to work properly in the Graph Explorer but 24 hours later we don't see the 'custom properties' on the profile card. https://learn.microsoft.com/en-us/graph/add-properties-profilecard

According to the response screenshot you provided, your custom attribute has been added successfully, but it is not displayed in the profile card.
You need to note that the profile card that allows customization is currently only applicable to Web applications. For example, Outlook Web App, OneDrive, SharePoint Office.com. The added custom properties are not visible in the Outlook client because this change is currently limited to the Web client experience.
