We have 3 pass through agents before, but now it is missing. We have a AD sync. The missing 2 pass through agents reside to on-prem.

If they are missing then you must confirmthe status. Are they uninstalled on prem or crashing?
Go to Microsoft Entra admin center > Identity > Authentication methods > Pass-through authentication
You’ll see the status of all registered agents. Check the last check in time and their statuses.
On the servers that used to host the missing agents open services.msc and start the Microsoft Azure AD Connect Authentication Agent if stopped.
Also check event logs on the DC to see what may have happened.

Check if details are available in Entra ID portal. entra.microsoft.com -> Hybrid management -> Microsoft entra connect -> cloud sync. Under user-in, click on pass-through authentication. It will provide the details of agent IP and status.

Based on your query, when using cloud sync, there is no issue with Pass-Through Authentication.
Please try to update your AD Connect regularly.
