Being charged recursive fee for non-existent service (Azure DB for MySQL flexible server)

Lately we noticed there were abnormal fee in our Azure pay-as-you-go subscription. And upon further inspection we found that we were charged for "Azure Database for MySQL Flexible server", which cost us ~US170/month, date back to almost a year ago.

When we check our service plan we cannot find such item. And I submitted 3 refund tickets (because it won't allow me to request refunds for older bills) and get refunded. HOWEVER, in my new bill I see "Azure Database for MySQL Flexible server" again, and now I can't even apply for a refund because "Expedited refund limit reached".

This is ridiculous, please help to fix this and refund. The fact that I need to subscribe to a support plan (USD29/month) to submit this request is even more ridiculous.
