Where can I submit a form for a Replacement Keyboard - Microsoft Sculpt Ergonomic Desktop keyboard

Key board works intermittently, I've searched everywhere and called every number but everything is a dead end. This is within the 3 year warranty of the product and want to get a replacement sent

However, Repair orders are already beyond of our scope here in Microsoft Community since we do not have the necessary tools. Thank you, I've been looking everywhere for this. However, I've clicked the option for "keyboard" and then the next field is to list "what keyboard do you have" but this dropdown menu doesn't provide any options and I cannot submit without it
