I keep getting errors trying to add a new server, or even run commands against the content database.
I am using a named instance but default ports (1433) on the sql server , all protocols are enabled (named pipes, tcpip) no firewalls blocking
ran checkdb and no errors were found , all sites are up but I can not do any maintenance/upgrades on databases
any ideas?
SO it says 2 patches are missing but when I try to install it says they are already installed.
Also it says my 2 content databases need upgrade but I can not upgrade them, I get an error "cannot be upgraded"

Please make the following troubleshootings to narrow your issue
1.Give the user SHELL ADMIN ACCESS on that content database, then try again.
2.Increase the permissions(dbowner) for content database and then check if issue solved.
3.Try to run the below cmdlet to upgrade all content database that still needs upgrade
Get-SPContentDatabase | ?{$_.NeedsUpgrade -eq $true} | Upgrade-SPContentDatabase
4.Try below cmd commands:
PSConfig.exe -cmd upgrade -inplace b2b -force -cmd applicationcontent -install -cmd installfeatures
Also try to clear the config cache and re-run the upgrade.

no joy, ran the update on all servers now I get different error trying to run config wizard

First, make sure that timer jobs are running on all servers.
Then please check following things:
What's missing?
$spprod = Get-SPProduct
$spprod.Servers | select ServerName, Products, InstallStatus
$spprod.Servers | ? { $_.InstallStatus -eq "InstallRequired" } | % { $_.RequiredButMissingPatches }
Are databases need update?
Get-SPContentDatabase | select Name, NeedsUpgrade
If all are set, please run the follow script:
PSConfig.exe -cmd upgrade -inplace b2b -force
Optional: -cmd applicationcontent -install -cmd installfeatures -installcheck -noinstallcheck.

According to my understanding of your problem, when you want to add a new server to an existing SharePoint 2016 farm, the following error will appear when running the Products Configuration Wizard:
Please following steps:
1) Exit the SharePoint Products Configuration Wizard.
2) Install the latest CU patch in your farm
3) Re-run the SharePoint Products Configuration Wizard.
If the problem still occurs, it is recommended that you view the ULS logs or Event Application logs to get the detailed error analysis report.
