How to show employee name and place by current date in excel

I want to get Employee name and his error type like fatel or non current by a previous date. It should show in tabular form dynamically.

    I want to display Employee Name and residence city by year.
    Like We want names of employee leaving in cities of Florida State current year.

    I tried to get result using Filter Function but it displayed all columns of the table instead of showing only names and City

    To my understanding, you may use PivotTable in Insert tab.
    I create a sample.
