" Why did my PC restart ? " - screen after SCCM re-image (OSD)

We recently started seeing this on Lenovo models and Virtual machines116925-whydidmypcrestart-thumb-1394467e5cbaf463efa20ca9f1.png lately.

The task sequence completes successfully, but this appears before the login screen. We are on SCCM Version 2103.

Steps tried so far to remediate:

  1. blocked Computer Configuration\Administrative Templates\Windows Components\App runtime\Block launching Windows Store apps with Windows Runtime API access from hosted content. using registry
  2. removed the following from unattend.xml

NetworkLocation=Work
SKippMachineOOBE=True
SKipUserOOBE=True

  1. Disabled telemetry & customer improvement program

Event viewer says -

The process C:\WINDOWS\system32\taskhostw.exe (PC123) has initiated the restart of computer PC123 on behalf of user NT AUTHORITY\SYSTEM for the following reason: Operating System: Reconfiguration (Unplanned)
Reason Code: 0x20004
Shutdown Type: restart

    Triple check you have removed these two lines in your unattend.

    <SkipMachineOOBE>true</SkipMachineOOBE>
    <SkipUserOOBE>true</SkipUserOOBE>

    I would even open a cmd prompt and inspect the unattend.xml that is layed down in WinPE before machine restarts.
