Forms - uploading an image does not change the background only the colours

Question

I have forms created and placed in a group.  I want to change the background image on one of them to make it more interesting.  When I click the + to create a custom theme, I upload the image.  The colours of the form change but the background image never displays.

I had the same issue with a form in My Forms but after three tries, the background image did change and has stayed changed.  I have successfully changed the background on another form in the same group.  I can change the theme on the form in question back to a solid colour.  If I go into the custom theme and click the trashcan, the icon changes so I can upload a new picture but I see no way to completely delete a custom theme and start over.

Can someone advise me as to what is required to be able to get the background image to display?  This function does not appear to work properly.

I know this will be a lot of work, but you can try to recreate some of your forms and check if there is any difference.

Thanks for all your help on this John.  Loading the same background to this one form, that is already in use on other forms just does not work as advertised.  At this point, I've spent more time on the problem than it's worth.  I really appreciate your efforts but I am going to give up on this working and move one.

Hi Ross,

Unfortunately, you can't delete a custom theme for a form. I am not sure if this is what you meant but to change the banner and button to custom colors, click the Paint Bucket icon. A field will open that asks for a hex color value. If you are unfamiliar with hex values, you can learn more and find them here. https://www.color-hex.com/

Thank you John for your attempt to assist.  I sincerely appreciate it.  I have changed the background successfully on many forms, it is only this one that it fails on.  Are you aware of a means to delete a custom theme for a form?  If I click the trashcan in customize theme, it only deletes the background but not the colour scheme it created.  

The link provided is the mechanism that I used to change backgrounds on all my forms.  Only this one form, which is multiple pages with lots of branching fails.

Have you tried clearing your browser's cache and then restarting it?

As what is mentioned on the link below, if you want to select another background image of the survey, select Customize theme, and then Upload Image.
