I have a Microsoft Form setup and one of the questions requires a file upload.
This has been working succesfully for months with no issues.
Suddenly the funcionality to upload a file is giving the following error:
Failed to upload your file.
Ive tried recreating the form, but this approach wont let me add the file upload question at all. Ive checked the folder in OneDrive and it creates a Question folder, but then deletes it for some reason
Please help as I have been through the form and tried all suggestions but it is not working still.

Sorry for the inconvenience the problem caused for you.
Based on your description, you can't upload files in Microsoft Forms. I tested it on my side, however, there was no problem.
You also mentioned that the OneDrive folder for Microsoft Forms will be deleted, could you please go to the recycle bin in OneDrive to check whether the file is there and who deleted the file?
And to check whether the problem is related to the tenant, may I know that whether your colleagues have the same problem on their side?

Are there issues with Microsoft Forms today? I am trying to create forms and it keeps saying something went wrong or it can't save my changes to refresh the page.
