Adding a Signature in Microsoft Forms

Dear Team,

Greetings for teh day.

We are using Microsoft forms to collect data from external parties. We would like to get their Signatures on the form supplied.

Is there any provision to collect the signature on the Form without the help of Power Automate.

We are using Office365 @ enterprise level.

Please share us your expertise on the above subject.

Thanks for the response.

While external users upload files are not supported in Forms for now, it could be very hard to collect the signatures. I find one user shared his idea on how to accomplish this with a workaround.

"What I ended up doing was adding a link to request files to a onedive folder. Then set up a flow that moves that file to a sharepoint folder and notify someone. The sharepoint folder I've moved it to has a lookup field to a list from the form responses. When someone fills in the document field for their name (lookup field), it adds it as an attachment to the item. This is the only workaround that I have found works."

Good day! Thank you for posting to Microsoft Community. We are happy to help you.

Based on your description, we understand that you would like to collect the names of users outside your organization in Forms.

There are currently three settings for collecting responses in Forms, only the first of which allows users outside the organization to fill out the questionnaire, but for privacy reasons, the names of the people who fill in the questionnaire are not automatically collected.

  Therefore, I think the best course of action is that you can add questions to the questionnaire to collect the names of the people who fill it out:

Along with the Name we would like to capture the user Signature as well. Appreciate if you share expertise on this.

Hi Abdul Basith Abdul Bari,

Forms app doesn't have a specific type of predefined question for Signature/E-Signature. A possible workaround is to add an Upload File for collecting Signature. Users can add their signatures in a Word, PDF, Image and upload it to Forms

Hi Ryan,

Firstly, Thanks for the prompt and clear response.

As we have to circulate the form to the external users (not within the organisation) we can't have the upload option in MS forms if the below option is enabled.

Kindly advise if there is a way to have the upload option with the below setup enabled.
