Azure Cloud IAAS template or questionnaire

Hi, Are there any templates or questionnaire brochure is available which would simply give options to fill for an application owner to tell the Azure team, how many servers are needed, how much storage, what network requirement is needed. Something like that? Once team receives the template they would know what to build in the infrastructure part.

For your requirement I suggest using CSV import feature where you can provide inputs and assessment gives you details about Azure suitability analysis, migration cost planning, and performance-based rightsizing.
