Microsoft Authenticator code rejected – Error 500121 despite correct setup

Hi everyone,

I'm having persistent issues signing in to my personal Microsoft account (<Mod remove PII> @live.se) using Microsoft Authenticator. I've followed all the correct steps:

Enabled two-step verification via account.live.com/proofs/manage

Added my account to the Authenticator app using the QR code provided

Verified that the app shows a 6-digit code that refreshes every 30 seconds

Synced time settings on my phone and ensured notifications are enabled

Despite this, when I try to sign in and enter the code, I consistently get:

Error Code: 500121 – “You didn’t enter the expected verification code.”

I've removed and re-added the account multiple times, tried different verification flows, and even attempted to use push notifications — which also fail to arrive.

It seems like my MFA configuration is broken or misaligned on Microsoft's end. Can someone from Microsoft please help reset my MFA setup so I can reconfigure it properly?

Thanks in advance, Jamal

It sounds like you're experiencing an issue with the passwordless sign-in process using the Authenticator app. One possible reason for not receiving the extra identity verification notification could be that you have an unanswered passwordless phone sign-in verification pending. In this case, the system may not send a new notification until the previous one is addressed.

To resolve this issue, follow these steps:

    1. Open the Authenticator app on your device.
    2. Check for any pending notifications and respond to them.
    3. After addressing any pending notifications, try signing in again to see if the extra verification step works as expected.

If the problem persists, it may be beneficial to check if your account settings are configured correctly or to reach out to your IT support for further assistance.
