MS Forms Upload Question - Failed to upload your file error

I'm new to MS forms and having issue with one of the forms shared with me. The Primary Owner has already moved to another team but has been me the admin access to the form.

MS Forms Setting = Only people in "My Company" can respond enabled.

I have already recreated the question but still keep getting the attached error.

Can you please advise what is missing from my end? What else should I check?

I am trying to submit a single JPG file which is less than 10MB in size. upload file error

On another note, could you also please advise because as part of my troubleshooting, I created a new question just to check if that will fix my above issue. However, when I do Preview, the form does not show the new field I created. It appears to still show the old upload field. Can you advise on this too, please.

Based on your description, I understand that you’re using Microsoft Forms with a question that allows respondents to upload a file, but when you try to upload a JPG file (less than 10MB), you see the error “Failed to upload your file.” You also mentioned that after creating a new question to test, the Preview still shows the old upload field instead of the new one.

    I tried to replicate this scenario on my side, including uploading files and editing questions, and everything worked as expected. The Preview always updated after changes. This suggests the issue might be related to the specific form or its configuration.

    To help narrow this down, could you please try creating a new form under your current account, add a question with the Upload file option, and check if the problem persists?
    Go to Microsoft Forms and select New Form

    Add a question using File upload, confirm the prompt to create a storage folder, and set the file type and size limits.User's image

    Click Preview and try uploading a small JPG file.

    • If the new form works fine, the issue is likely tied to the original form’s ownership or settings.
    • If the problem continues, it may indicate a broader configuration or policy issue.
