Server readiness shows Unknown state

Hello All,
I have enabled discovery and then assessment for a set of servers in Azure Migrate project. I'm getting the "Azure VM readiness" as Unknown for a few servers. It has been 10+ days since I have created the assessment.
I could not find the reason about why these VMs are in "Unknown" state, hence require help to identify this and try fixing the same.

In Azure Migrate, a "Readiness Unknown" status after 10 days almost always indicates that the Azure Migrate appliance is unable to collect specific metadata from those servers, rather than a lack of time. Since you have already waited the required duration for a performance-based assessment, the issue is likely a data availability or connectivity problem.
The portal usually stores the specific error message:
- Go to Azure Migrate > Servers, databases and web apps.
- In the Discovery and assessment tile, click on the total count of Servers (under "Discovered servers").
- Look for a column named Discovery issues or a notification at the top that says "Resolve issues". Click it.
- This will show you a list of "Affected objects" and an Error ID. Clicking the Error ID will give you the exact reason (e.g., "Insufficient permissions," "WMI connection failed," or "Guest OS not identified").
Based on common scenarios for "Unknown" readiness:
- Missing Guest Credentials: If the appliance cannot log into the server, it cannot determine the Boot Type (BIOS vs. UEFI) or the exact OS version. Without these, Azure cannot confirm if the VM is "Ready." To resolve this Open your appliance configuration manager (local web UI) and ensure you have added valid Guest Credentials and they are successfully validated against those specific VMs.
- Unsupported or Unidentified OS: If vCenter/Hyper-V reports the OS as "Other" or the appliance can't read the OS string, the status remains "Unknown." Ensure VMware Tools or Hyper-V Integration Services are running on the target VMs. This is how the hypervisor passes OS info to the appliance.
- WMI/SSH Connectivity: The appliance needs to reach Windows VMs via WMI (Port 135/445) and Linux VMs via SSH (Port 22) to pull metadata. Check the local firewall on the target servers to allow traffic from the Appliance IP.
- Assessment Sizing "Gaps": If you chose "Performance-based" sizing and the server was powered off for a significant portion of those 10 days, the appliance might not have 100% "Performance Coverage." Change the assessment properties from "Performance-based" to "As on-premises" and click Recalculate. If the status changes to "Ready," you know the issue was missing performance data.
If you believe you have fixed the issue (e.g., updated credentials or opened a port):
- Go to your Assessment in the Azure Portal.
- Click the Recalculate button at the top. Note: Assessments are snapshots; they do not auto-update when you fix things. You must click Recalculate to see the change.
- Wait about 15–20 minutes for the job to complete.

- Are all VMs experiencing this issue, or is it just a few specific ones? Ans: Out of 53, 7 servers are having status as "Unknown" and "Ready with conditions"
- What OS is installed on the servers? Ans: Windows 10, Windows 11 and Windows Server 2016 Standard
- Have you reviewed the Errors and notifications blade for any relevant issues? Ans: I tried searching for the errors, but couldn't find those on the portal.
- Can you confirm if the SQL Server instances are categorized correctly? Ans: Domain credentials to the SQL servers are provided in the appliance. Out of 4 SQL servers, we see 1 has this issue.
It would be great to get onto a call and check upon the issue. Please let me know how shall we connect?
