Formatting disappeared when document was downloaded

Question

A friend sent me a document that she created using columns. When I downloaded the document, the formatting was gone, the footer information was missing and the pagination was gone. Does anyone know how to stop this from happening??

I use Google chrome but don't know if that matters.
