FLTMGR.sys BSOD after Windows Update 24H2 on Optiplex 7060

Hello everyone,

This is the third time I am posting about this since I haven't been able to resolve it yet. When I try to update from 23h2 to 24h2, I keep gettting the black/blue screen stating that Stop Code: SYSTEM_THREAD_EXCEPTION_NOT_HANDLED (0x7E), What failed: FLTMGR_SYS.

I have tried uninstalling some drivers that cause memory integrity to fail which I read from some forums was the case for them. I also disabled and uninstalled SentinelOne because I heard that third party applications might affect it. I even switched from RAID on to AHCI boot to see if that would make any difference. I have uninstalled some dell applications that might use legacy drivers to cause that issue but every single time I have to face the same error.

I would really appreciate it if someone can guide me in the right direction with this issue as I feel like I keep going around in circles while trying to troubleshoot it.

FLTMGR.sys BSOD is usually caused by driver issues. The steps you took with Memory integrity is highly appreciated.

You may try cleaning old hidden devices by following steps:

Open Device Manager
On menu bar, choose View > Show hidden devices
Now please expand and browse through every title, you will see some devices are grayed out
Uninstall all grayed out devices
Reboot your computer

Then you may try updating your system again.

If the issue persists, please try performing an In-place upgrade. Your system will get the latest 25H2.

Hey Lucus,

I uninstalled all the hidden drivers but it still does causes that blue screen. I will not be able to do an in-place upgrade as this is an issue network wide on my workplace so it will not be feasible to do that everywhere since different departments have different applications and setups they would require. I won't be able to risk not losing data from the computers on the network.

I have uploaded the two upgrade logs for everyone to see in case you find something there that might be pointing to the issue.

An in-place upgrade is designed to be safe and to preserve your personal files, applications, and settings. It only replaces the system files, which helps fix issues while keeping everything else intact.

I will try looking into alternative solutions that feel safer and more comfortable for you:

Scan your system with these commands using Command Prompt (Run as Administrator):
DISM /Online /Cleanup-Image /RestoreHealth sfc /scannow

Reboot the computer.

Make a system restore point.

Remove Intel drivers from Programs and also Device Manager if they're still existed.

In case removing Intel drivers causes system error, please revert from a restore point.

Reboot the computer.

Try upgrading again.

Hello Lucus,

The computer is still giving me that error after attempting to do the steps that you gave me. Both of the commands showed no errors so I thought the upgrade would work this. But no luck. Please advice.

I know it's a bit inconvenient, but I would suggest you performing a clean install. Your system will directly get the latest 25H2.

Disclaimer: At this point, we have exhausted all troubleshooting and I recommend that we try to perform a clean install to get your computer back into a working condition. Please ensure that you backup any important data, including Documents, Pictures, Videos, and more.

That is not feasible to do since a lot of machines have this problem and they all have different ways that they are setup with their own applications and programs. I will keep looking into the issue. Thank you for your help.

Hey Jan!

For some reason, the minidump folder was empty every time I tried to do the update to see if something will come up. I changed settings on advanced system settings but every time it was empty. I will attach links to the setuperr.log and the setupact.log files to see if might find something there. I will keep trying to see if the minidump files will generate and will immediately send those as well once available.
