Customising Response Receipts

I would like to be able to include content which readily allows me to recognise for which form I have compelted a response for. For example, on a monthly basis I have several clients I have to complete a form for, which goes through the level of satisfiaction and any issues experienced for that client. When I complete the form for each client I receive a generic email confirmation but this does not contain any refernce to each of the clients and for which month it was completed for. I would like to be able to view both the client's name and the month completed for on each response reciept so I can file these away accordingly and keep track of those not completed. Is it possible to tailor the response receipts to include these details which are added at the point of completing the form?
