Users unable to access Microsoft Form (404)

Hello,

I have a form that allows users to enter information and upload files. This form triggers a power automate flow.

The form is limited to users in my organization, many different people complete the form, and most users will have to complete the form multiple times.

A user from my organization is telling me that they tried to open the form and got a 404 error. I've heard this issue from others, but the links work when I test them.

Is there any known issue that causes trouble with form links?

I included the use information in case the issue is related.

    Thank you for the detailed explanation. Since the form works when you test it but some internal users are receiving a 404 error, there are a few known scenarios that can cause this behavior. 

    1. Safe Links rewriting (Email or Teams) 

    If your organization uses Microsoft Defender Safe Links, URLs shared through email or Teams may be rewritten into a long format such as: 

    "https://…safelinks.protection.outlook.com/?url=… "

    This “safe link” rewriting could be identified as a common cause of 404 errors when accessing Forms. In these cases, the recommendation is to provide the original Forms URL as plain text, rather than as a clickable embedded hyperlink. 

    To resolve this, please follow these steps: 

    • Send the user the original Forms URL pasted as plain text on its own line. 
    • Ask them to copy and paste it directly into the browser address bar instead of clicking it from the message body. 
    1. Stale or glitchy sharing lin 

    Regenerating the sharing link can often resolve inconsistent access issues. To do this, please follow these steps: 

    1. Open the form’s Share settings. 
    2. Toggle the “Who can respond” option away from and then back to your desired setting (for example, internal users only). 
    3. Copy the newly generated link and resend it. 
    4. Ask the affected user to open it in an InPrivate or Incognito browser window. 

    If the issue persists, duplicate the form and share the duplicate’s link. This can sometimes clear hidden configuration inconsistencies. 

    1. File upload dependency on SharePoint or OneDrive 

    If your form includes file upload, the uploaded files are stored in SharePoint or OneDrive for work or school: 

    • Personal forms store files in the form owner’s OneDrive. 
    • Group forms store files in the associated SharePoint document library. 

    There are known scenarios where SharePoint or OneDrive policies can interfere with file upload functionality, including: 

    • Restricted sharing link settings 
    • Limited access or lockdown features 
    • Unmanaged device restrictions 
    • Conditional access or other SharePoint access controls 

    Although these usually surface as “access denied” or upload-related errors, users may perceive them as the form being broken or returning a 404. 

    It would be helpful to check whether the affected users have anything in common, such as device compliance status, network security controls, or specific SharePoint access restrictions. 

    1. Power Automate connector check 

    While this typically would not cause a 404 on the form link itself, it is still worth confirming: 

    • The form still exists in the same location where the Power Automate flow expects it. 
    • The flow is still properly connected to the correct Form ID. 

    If these checks are correct but the issue still persists, the best next step would be to have your IT administrator open a support ticket with Microsoft via the admin center. A technical support engineer can then initiate a remote session, review backend settings, and run diagnostics. If needed, they can escalate the case to specialized engineering teams who are well-equipped to perform advanced troubleshooting and provide targeted solutions.
