Based on your description, since you are able to access other forms and only one specific questionnaire is affected, this issue is likely related to that individual form rather than your account.
Please try the following steps:
Step 1/ Access the form in a different way
- Sign out of Microsoft Forms completely.
- Open a private/incognito browser window.
- Go to https://forms.office.com and sign in again.
- Try opening the affected form.
Step 2/ Try a different browser or device
If possible, please:
- Try another browser (Edge, Chrome, Firefox).
- Or try accessing the form from another device.
-> This helps determine whether the issue is local (cache/session related).
Step 3/ Check if the form was moved or ownership changed
If this form was shared or transferred:
- Confirm whether you are still listed as the owner.
- Check if the form exists under:
+ My forms
+ Shared with me
+ Or within a Microsoft 365 Group (if it was created in a group).
-> If the form was created in a group that was deleted or if ownership was removed, access to that specific form may be impacted.
Step 4/ Provide additional details
If the issue persists, please share and let me know:
- A screenshot of the full error message
- Does the error appear immediately when clicking the form?
- Or after it starts loading?
- Does the same error appear on all devices?
