Problems with Voice Access for Windows 11 consistently mistyping a word; difficulties with punctuation commands

I use Voice Access for Windows 11 regularly for dictation in Word. I find it superior to Windows 10's Speech Recognition for moving around the screen. But infuriatingly inferior for dictation. I'm using unusual academic vocabulary and it is excruciatingly slow to correctly reproduce abstruse words, even when I "add them to vocabulary". It will sometimes offer them as part of its correction list, but almost never as its originally dictated word. I'm using a headset, so clarity should not be an issue.

It also has significant problems with common words. The most problematic is that for the last couple of months, it now always types "!the" [minus the quotes] whenever I dictate "the" [minus the quotes]. I have "corrected" it hundreds of times. But still it always regurgitates !the, though it offers "the" [minus quotes] on its list of possibles. This is horribly time consuming. Any suggestions as to how to persuade Voice Access not to type !the?

I also have problems with punctuation. "question mark"/insert question mark is never recognised. It always types the word literally.

Voice Access will open quotations and parentheses - but always fails to close them, with the manage that it cannot close them because they are not open.

Any suggestions gratefully received!

    I understand how frustrating this is, much of what you’re seeing comes down to current design limitations. Voice Access in Windows 11 is primarily designed as a control and navigation tool, not a full replacement for the old Windows Speech Recognition dictation engine. While it supports dictation, adding words to the vocabulary mainly improves correction suggestions, not first‑pass recognition. That’s why uncommon or technical terms rarely appear correctly on initial dictation, even after being added, and instead show up only in the correction list.

    Practical workarounds that you can try:

    • Using “type <word>” or spelling mode for problematic words.
    • Dictating punctuation inline (for example, “how does this work question mark”).
    • Using Voice Access for navigation, but switching to Win + H (Voice Typing) or Word’s Dictate feature for longer dictation, as those tools handle vocabulary and punctuation more reliably.

    If this behaviour (especially the “!the” issue) started recently, it’s worth reporting it directly via Feedback Hub (Windows + F) under Accessibility → Voice Access.
