I understand how frustrating this is, much of what you’re seeing comes down to current design limitations. Voice Access in Windows 11 is primarily designed as a control and navigation tool, not a full replacement for the old Windows Speech Recognition dictation engine. While it supports dictation, adding words to the vocabulary mainly improves correction suggestions, not first‑pass recognition. That’s why uncommon or technical terms rarely appear correctly on initial dictation, even after being added, and instead show up only in the correction list.
Practical workarounds that you can try:
- Using “type <word>” or spelling mode for problematic words.
- Dictating punctuation inline (for example, “how does this work question mark”).
- Using Voice Access for navigation, but switching to Win + H (Voice Typing) or Word’s Dictate feature for longer dictation, as those tools handle vocabulary and punctuation more reliably.
If this behaviour (especially the “!the” issue) started recently, it’s worth reporting it directly via Feedback Hub (Windows + F) under Accessibility → Voice Access.
