When I click the link to the form it says the request is blocked. I have had access to this form for over a year and other co workers are able to access it.

    Dear @Aaron Fischer,
Thank you for posting your question in the Microsoft Q&A forum. I understand that you are unable to open a Microsoft Form on your phone and see a message saying “the request is blocked”, even though you have accessed this form before and your coworkers can still open it.

Based on this behavior, the form itself is still working correctly. When a form opens for other users and on other devices, this usually means the access is being blocked only on the specific phone or network, rather than an issue with the form or your permissions.

This can happen due to mobile device security settings, the network you are connected to, or the browser used on the phone.

To better assistance, could you please confirm following information:

Are you opening the form in a mobile browser, or inside an app such as Teams or Outlook?

Does the form open correctly on a work computer or another device?

Are you connected to company Wi‑Fi, VPN, or mobile data when the issue occurs?

If possible, could you share a screenshot of the exact message shown on your phone?

In the meantime, you may try the following:

Copy the form link and open it directly in a mobile browser such as Edge, Chrome, or Safari

Switch between Wi‑Fi and mobile data and test again

Make sure you are signed in on your phone with the same work account used previously

If the issue continues after these steps, your company IT administrator may need to check whether mobile device or security policies are blocking access on that phone.

Please understand that our initial response does not always resolve the issue immediately. However, with your help and more detailed information, we can work together to find a solution.

I truly appreciate your patience and understanding. If you have any further questions or need further clarification, please feel free to reach out. I'm looking forward to hearing from you.

    Thank you for your cooperation.

Since coworkers can still open the form, the issue is likely related to device or access controls rather than the form itself. Try these steps on the phone:

    1. Use a different network
      • Switch between Wi‑Fi and mobile data.
      • If on a corporate Wi‑Fi or VPN, disconnect and try again. Conditional access or network reputation can cause requests to be blocked.
    2. Try another browser or the Microsoft Forms/Office app
      • If using an in‑app browser (e.g., from Teams or Outlook), copy the form link and open it in a standalone browser (Edge, Chrome, Safari).
      • If you have the Office/Forms app, sign in with the same work account and open the link there.
    3. Check for device access restrictions
      • If the organization blocks access from unmanaged or untrusted devices, the request can be blocked even if the form itself is fine.
      • Sign in to another managed device (for example, a work PC) and confirm the form opens there.
    4. Ask the Microsoft 365 admin to check for blocks
      • An admin (global or security admin) should:
        • Check if the form or the owner has been flagged for phishing in the Microsoft 365 admin center → Message center under the Prevent/Fix: Microsoft Forms Detected Potential Phishing notification.
        • If the form is listed and is legitimate, select Unblock for that form.
        • If the user (form owner) or your account is listed as blocked, select Unblock for that user. It can take 30+ minutes for restrictions to clear.
    5. If the form was previously blocked and then edited
      • If the owner removed flagged keywords, the block may have been lifted automatically. In that case, no admin action is needed, but device/network restrictions can still block access on specific phones.

If none of the above works, share the exact error text and have the admin verify there are no conditional access or SharePoint/Forms restrictions on mobile or unmanaged devices.
