Use the built-in preview and question settings in Microsoft Forms to verify and correct the issue.

1. Confirm that questions are actually added to the form
   • In the form editor, select Add new and choose a question type such as Choice, Text, Rating, or Date. For more options, select More question types (e.g., Ranking, Likert, File upload, Net Promoter Score®, or Section).
   • Ensure each question has content (title/text) and is not left blank.
2. Check that questions are not accidentally deleted
   • If a question was deleted, it and its response data are permanently removed and will not appear in preview. Recreate any missing questions by selecting Add new again.
3. Use the Preview feature correctly
   • On the web: select Preview to see how the form will look. On mobile, select the mobile icon in Preview to see the mobile layout.
   • In OneNote for the web or Forms for Excel scenarios, select Preview to see how the form will look on Computer or Mobile.
   • Answer the questions in Preview mode and select Submit to verify that questions behave as expected. Select Back to return to editing.
4. Verify section organization
   • If Section is used, ensure questions are placed in the correct section and that navigation between sections in Preview is working (scroll or move to the next section so all questions are visible).
5. Check text formatting only (does not hide questions)
   • Formatting options like Bold, Italic, Underline, Font color, Font size, Numbering, and Bullets affect only appearance, not visibility. If questions are present but hard to see, adjust formatting instead of assuming they are missing.

If questions still do not appear after confirming they are added and not deleted, create a new test form and add a few simple questions to see if they show in Preview. This helps determine whether the issue is with a specific form or the Forms environment.
