In Microsoft Authenticator, gray and inactive account tiles usually indicate inactive accounts created by other applications for single sign-on. These inactive accounts do not need any management and can be safely ignored.
They are not meant to be “re-activated” inside Authenticator. To actually sign in to those accounts, use the normal sign-in flow for the app or service (for example, Outlook, Teams, or the website for that account). If the account still works, it will prompt for sign-in or MFA again and can recreate an active entry in Authenticator if needed.
If the gray entries are not needed, they can simply be left as-is and ignored.
