I created a form for questions to be answered. This form is used by my crew to log events. The link and the form with questions are still available. I am not able to see the form to obtain the responses or to make changes to the form. The form is not in deleted files or groups. The ability to view this form as an administrator has completely disappeared. What can I do to recover it?

I think the form didn’t vanish…its owner‑ptr slipped off; open OneDrive→Apps→Microsoft Forms and tap the tiny json to pull it back, and if even that file’s gone, only an admin restore of the original acct + support re‑bind will fix it.

Hi, are you using a work or school account when you created the form?

Go to Microsoft Forms and sign in, then check My forms and Shared with me tabs. If not there, open the group in Outlook or Teams where it was created and access the form from that group, since group forms are owned by the group. Also check OneDrive Recycle Bin and restore any related files, then go to the admin center and review user accounts or ownership if the original owner was removed. These will help you locate or restore access to the form and its responses.
