Microsoft Forms Add-in not uploading form(s) Quizzes in Microsoft PowerPoint.

I have tried everything I can to get A quiz into PowerPoint using Microsoft Forms. but I cannot get Microsoft forms to upload in power point. I tried selecting the Add-on button, using the forms button. Checked the settings. I checked for latest updates, etc Its just shows its loading but doesn't do anything. Is there a way I could get assistance on this one? Thanks in advance.

Hi Jesse,

Thank you for sharing the details.

First, please ensure that your account and PowerPoint version meet the requirements. The option to insert a form or quiz into PowerPoint is available for Office 365 Education and Microsoft 365 Apps for Business users running PowerPoint for Microsoft 365 version 1807 (Build 16.0.10711.20012) or later on Windows. You must also be signed into PowerPoint using your work or school Microsoft 365 account.

If the Forms option appears but does not load or insert properly, please check whether web add-ins are blocked:

  1. Open PowerPoint.
  2. Go to File > More > Options.
  3. Select Trust Center > Trust Center Settings.
  4. Choose Trusted Add-in Catalogs.
  5. Ensure the following options are not selected:
    • Don’t allow any web add-ins to start
    • Don’t allow web add-ins from the Office Store to start

After adjusting these settings, restart PowerPoint and try inserting the quiz again.

If the Forms add-in is still stuck on loading, the issue may be related to your organization’s Microsoft 365 configuration. If you do not see Forms under the Insert tab, or if it fails to load, your IT administrator may need to verify that Forms and Office Store add-ins are enabled.

An admin can enable access by going to: Microsoft 365 Admin Center > Settings > Org settings > User owned apps and services, and enabling Let users access the Office Store. Note that changes may take some time to take effect.

Alternatively, the admin can deploy only the Forms add-in by following: Go to Microsoft 365 Admin Center > Settings > Integrated apps > Add-ins > Deploy Add-In > Choose from the store > Search “Forms for PowerPoint” > Add > Assign to users > Save.

As a workaround, you can also try using PowerPoint for the web:

  • Open your presentation online
  • Go to Home > Add-ins > Forms
  • Insert the quiz using its URL

PowerPoint for the web supports adding Forms content by entering the form or quiz URL and choosing either Present for interactive meetings or Collect responses offline.

If the issue persists after these steps, I recommend contacting your Microsoft 365 administrator to raise a support request through the Microsoft 365 Admin Center. The issue may be related to tenant-level settings, add-in deployment, or account authentication.
