Based on your description, this behavior is probably not related to the configuration of your form. There is currently an issue where email notifications for new replies in Microsoft Forms are not delivered as expected. This has been observed with several customers and in different regions, and there is a significant increase in related support requests.
Microsoft has acknowledged the issue and is actively investigating it.
Summary of the effects: Users do not currently receive email notifications when new responses are submitted through Microsoft Forms. Current status: The situation continues to be reviewed and analyses are underway to determine the cause and restore normal notification functionality. Currently, the problem can continue to occur sporadically.
What you can check on your site: While the investigation is ongoing, it's a good idea to verify that responses are still being captured in the form, control user accounts and access rights in the Microsoft 365 admin center, and identify any automated processes or integrations with high activity. Additionally, it can be helpful to check recent login and audit logs for unusual activity.
Possible interim solution: Alternatively, if these notifications are critical to your workflows, you can use Power Automate to configure your own email notification when new replies come in. This way you can ensure that you continue to be informed.

Sign in to Microsoft 365 Admin center. Navigate to Health > Service health > In the Reported issues tab, Select Report an issue.

Additionally, I strongly recommend submitting a support request to Microsoft as soon as possible. This issue may require deeper technical investigation that only Microsoft’s support team can provide. A technical support engineer can perform a remote session to investigate the situation, verify the backend configurations, and run any necessary synchronization tools to resolve the problem. If the issue requires further attention, they can escalate it to a specialized team for deeper analysis.
