Organization Identity Validation blade is blank despite Artifact Signing Identity Verifier role assigned (EU org)

Question

Environment:

• Artifact Signing (Trusted Signing) account: Kilbirivi, region Poland Central
• Subscription type: pay-as-you-go (not free/trial/sponsored)
• Organization based in the European Union (Estonia)

Problem: The Identity Validation blade has no document upload field. I cannot progress on a rejected organization identity validation.

What I've already verified:

• I hold the Artifact Signing Identity Verifier role, assigned to my own signed-in account, at both scopes: "This resource" (the signing account) and "Subscription (Inherited)".
• I am also Owner on the subscription.
• Signed out fully and re-signed in via a fresh InPrivate session (new token): blade still blank.
• Confirmed the subscription is registered for the Microsoft.CodeSigning resource provider.

History:

• A prior organization identity validation request was rejected asking for "additional document(s) dated within the past 12 months," but the request page showed no examples and no upload control.

Questions:

1. Why would the Identity Validation blade stay blank when the Identity Verifier role is confirmed at the correct scope on my signed-in identity?
2. Is there an existing failed/locked validation request on this account blocking new ones, and if so how do I clear it?
3. For an EU (Estonian OÜ) organization, what document satisfies "dated within the past 12 months": is a current-dated business-register extract (e-äriregister) accepted, and in what format/size?

Just to be clear, here is what I am looking right now:

a Failed identity validation request, showing a blank error of "Please provide additional document(s) dated within the past 12 months to verify your organization. Examples of verification documents are as follows:" with no documents listed as exapmles, nor any way to submit a document.
