I am building a form but when I go to preview it, it has Upload File which I did not add, it is required and will not let me delete it. I don't want to upload a file ..I want to build a simple form.

There is nothing that will allow me to delete the question or display if it is required or not (the other questions will let me do both). See screen shot.

Based on your screenshot, it appears that you are creating a form from SharePoint, rather than using Microsoft Forms directly.

The behavior you are seeing is expected if the form is being created from a Document Library in SharePoint. A Document Library is designed to store files, so the Upload files field is a required part of the submission process and cannot be removed. In this scenario, the form is intended to collect files from users, which is why the Upload files question is automatically included.

Please note that the Forms experience in SharePoint is different from Microsoft Forms, even though they may look similar.

If your goal is to collect information without requiring users to upload files, you can consider the following options:

Option 1: Create the form using a SharePoint list library

If you already have a SharePoint List, you can use it to create the form.

Otherwise, you may create a new SharePoint List, provided you have the necessary permissions.

Select Forms > Create a new form.

Forms created from a SharePoint List don't require an Upload files field, allowing you to collect information without requiring users to upload documents.

Option 2: Create the form in Microsoft Forms and share it through your SharePoint site (Recommended)

For your case, I recommend creating a new form directly from Microsoft Forms, then adding only the text, choice, date, or other basic questions you need. This should avoid the required Upload file field shown in the current screenshot.

After creating the form, you can make it available through SharePoint by adding the form link to your SharePoint site or embedding the form on a SharePoint page for easy access.

Additionally, Microsoft Forms allows you to export responses to an Excel file, making it easier to review, analyze and manage the collected data. If needed, you can save the exported Excel file in SharePoint so that it can be accessed and managed from a central location.

Just a few more questions. If I create a list, can the results of the list be fed into an excel sheet? Honestly that is my ultimate goal. The next question is I actually would have preferred to have a Teams form that can be saved to an Excel sheet on SharePoint but all the research I have done says it cannot be done. I am new to the MS suite and have worked with Slack and Google products whereby this is not an issue (including doing coding in the Google Sheet versus having to use Power Automate).

Thank you so much for sharing more details. Certainly, both scenarios are supported.

1/ Microsoft Lists can be exported to Excel and the workbook can be refreshed to pull the latest updates from the list. However, changes made directly in Excel will not sync back to the Microsoft List.

2/ For Microsoft Forms, responses can be stored in an Excel workbook. If the form is created from a Teams channel or SharePoint, the Excel file is saved in the associated SharePoint location, and new responses can be reflected in that workbook.

3/ Based on your requirement, I would recommend creating the form from Teams or SharePoint, collecting responses through Microsoft Forms, and then using the generated Excel workbook.

If you need the data to be written into a specific existing Excel file or follow a custom format, Power Automate would be the best option to automate that process.

Thank you for sharing your concern.

The Upload File item in Microsoft Forms is a question type that can be added to a form and marked as required. If the question exists in the form, it should normally be possible to remove it from the form editor.

To help determine whether this is a display issue or an existing question in the form, please try the following steps:

Step 1: Refresh the form editing session

Close the Preview window.

Return to the form editing page.

Refresh the browser page.

Open the form again in edit mode.

Check whether the Upload File question appears in the list of questions.

Preview the form again and see whether the issue persists.

Step 2: Check the upload file question

Open the form in edit mode.

Locate the Upload File question, if visible.

Select the question to open its settings.

Check whether the Required option is enabled.

Turn off Required if it is enabled.

Select the Delete question (trash bin) icon to remove the question.

Open Preview again and verify whether the question has been removed.
