Do you mean you are using Microsoft 365 apps now, but there is no option for Design Ideas?
I suggest you go to PowerPoint > File > Account > Product Informtaion, then please take a screenshot as following image, and share with us. I would check the version name, version number and update channel.
Actually, Office 2019 does not include this feature, this is a subscriber-only, Microsoft 365 feature.
- If you do not find Design Ideas button in PowerPoint for Microsoft 365 apps, please make sure you are not using Office 365 Germany Germany.

As your image above shown, the Office on your machine becomes Office Professional Plus 2019, I need your help to run commands to check the activation information of Office, this action could check whether there is any Office 2019 registration information that may cause this issue.

Your second image shows that there 2 versions of Office activation information on your machine.
One is Office Mondo which uses KMS technology that is not suitable for Microsoft 365 apps activation.
Another one is Microsoft 365 subscription which is not activated now.
It's recommended to uninstall the key for Office Mondo, we need to avoid its interference with the activation of Microsoft 365 apps. If you need the some apps included in Office Mondo but do not include in your Microsoft 365 subscription, it's better install Office Mondo in a virtual machine.
Please continue to enter following command in Command Prompt which is run as Administrator.
cscript.exe "%ProgramFiles%\Microsoft Office\Office16\ospp.vbs" /unpkey:XQBR2
Then, please close the Command Prompt, reboot your machine, open one Microsoft app to have a check.
If it needs to be activated, please log your licensed account for Microsoft 365 apps.

If this steps above do not work, I would suggest you use Microsoft Support and Recovery Assistant to uninstall Office completely.
And then, log your licensed account into Services & subscriptions, download your Microsoft 365 apps and reinstall it on your machine.
All we need to do is to make sure that there is only the Microsoft 365 app, and also make sure it is activated. Then the Design Ideas icon could be back.
