Compute figure selected row of one/two column in Power BI

Question

I would need people help me in power bi. I need to create new column based on table below to compute "Total participants"; if firstvalue will compute the same "Total participaint" group by health facility; but if MAX will compute "Total participants" of MAX minus "Total participants" of firstvalue group by health facility.
