Create a entry system using Powerapps connected to Google spreadsheet and give visual

Hi Microsoft, I'm Haziq from Malaysia. I want to create an entry system where the data can be transferred to Google spreadsheet (or any cloud database) using powerapps. Is it possible and if yes, how can i do it?

Attached is the example on how the entry system will looks like.
